Hope for the Plateau Midface Syndrome after Midface Surgery

What is the Plateau Midface Syndrome?

The “plateau midface syndrome” is an unofficial name that Dr. Steinsapir uses to aptly describe in a few words this constellation of changes after aggressive midface surgery.  Those who seek help for this problem often wonder if they are experiencing zygomatic arch erosion – erosion of part of the cheek bone or so-called temporal wasting.  Dr. Steinsapir has carefully studied these changes.  What is going on is the result of surgical trauma to the fat that cushions the temple area and extends to the edge of the orbital rim.  This fat is damaged by aggressive midface surgery where the surgeon tunnels into the midface from the hairline or above the hairline.  It is Dr. Steinsapir’s opinion that surgeons who cause this problem likely do not fully understand the damage caused by their surgery.  The effect is unmistakable.  The temple is hollowed, the bone of the zygomatic arch which was previously hidden by the temple fat becomes exposed.  This create a skeletonized look.  Because this area looks flat, Dr. Steinsapir has adopted the term Plateau Midface Syndrome to describe the condition.

Another feature of this syndrome is loss of fat at the edge of the eye area.  This area at the side of the eyes should have a subtle layer of fat.  This fat helps to isolate the eye aesthetic area from the temple aesthetic area.  The importance of this cushion of fat is critically in our day-to-day interactions with other people.  Gaze scan studies have demonstrated that this cushion of fat helps to hold conversation gaze of the person you are talking to on the eye.  When you are aware that the person speaking with you is looking into your eyes, you know they are interested in what you have to say.  However, when this fat is damaged by midface surgery, the gaze is no longer held by the eyes.  The gaze of the person speaking with you easily slips off to the temple area.  The effect is the awareness that people who you speak with are not interested in you or what you have to say.

Facial recognition and interpretation is a complex brain process, so the person looking at you will not be able to describe the issue without training and medical insight, but their brain will still have trouble making sense of your face, and they will lose their focus on your eyes and further lose interest in what you are saying.  This is not intentional on their part, but the result of complex neurological functioning.  When you experience this complication, what you say will not seem important to others because your eyes will no longer help you deliver your message.  The change in your interactions with others may even cause you to become severely depressed.

FDA Warnings Against Lipodissolve False Advertising

If you are considering getting lipodissolve, also called mesotherapy and liposhape, please first consider the recent FDA warnings issued against this untested, unproven procedure.  In April, 2010—shortly after our article warning readers that lipodissolve has not been proven safe—the FDA contacted six different medical spas in the US to warn them not to make misleading claims about lipodissolve.  Lipodissolve and the other catchy names it goes by are becoming popular trends in some circles, but the procedure raises ethical questions against those who persist on providing it despite lack of medical support.

What is Lipodissolve?

Lipodissolve and its many other titles involves injecting chemicals into locations where one hopes to lessen the amount of fat present.  There are many chemicals and combinations of chemicals that may be injected into the body, and the most common two are phosphatidylocholine and deoxycholate.  The medical spas and even some physicians who advertise this procedure call it safe and effective; some even claim it is better than liposuction.  This is dishonest at best, because lipodissolve has not been demonstrated to be safe or effective.  Because there is not enough scientific basis to medically sanction this procedure for human use, it is not FDA-approved.  Furthermore, the reported complications make it unlikely that the FDA will approve it anytime soon, and they probably won’t ever.

What Are the Risks of Lipodissolve?

With such little scientific knowledge about lipodissolve, we cannot quantify the risks of lipodissolve, but those receiving it have experienced complications such as scarring, skin irregularities, blood vessel damage, and subcutaneous nodules that cause pain.  Perhaps even more dangerous than these reports is the fact that those who insist on offering lipodissolve may have little respect for other scientific and medical safety conventions.

Without a broad and precise understanding of the exact risks and effects of lipodissolve, it is virtually impossible to administer the treatment using the amount of control needed to deliver the best results, and it is also difficult to know the best way to treat complications. Science is not on the side of lipodissolve injectors, so the best a practitioner of lipodissolve can do is guesswork.

What Warnings Did the FDA Give?

In April we saw that the FDA gave warnings to the following spas:

The FDA takes false advertising seriously, as it should, because misleading the public about their health is a serious and dangerous offense.  In these cases, spas and websites were reprimanded for making unsupported claims.  Take, for example, the letter that went to All About You Med Spa that included this statement: “The claims made for your lipodissolve products are false and misleading in that they are not supported by substantial evidence or substantial clinical experience.”  The FDA further instructed the above businesses to cease their unproven and misleading claims, and to notify the FDA within 15 work days of how they are correcting their violations.

What Are the Alternatives to Lipodissolve?

Lipodissolve has been touted as a safe and nonsurgical alternative to liposuction, but the truth is that liposuction is the safe, proven, and effective alternative to lipodissolve.  Liposuction has an established and respected reputation for a reason; as a body-sculpting technique, it improves the distribution of fat across your body, and it has been tested, studied, practiced, evaluated, and demonstrated to be a very valuable and effective treatment.

Help for Ptosis after Blepharoplasty

Eyebrow and eyelid ptosis is the most common reasons people seek revision eyelid surgery after blepharoplasty.  In most cases, this is not an urgent concern or an emergency, although it is still important and can be a source of disappointment following surgery.  These issues are relatively common after eyelid surgery.  Some degree of eyebrow ptosis is normal and occurs in 100% of cases.  It becomes a problem when it in someway reduces the apparent benefits of the surgery.  True upper eyelid ptosis following eyelid surgery is more complex.

What causes eyebrow ptosis after eyelid surgery?

There are a couple of reasons why you may be experiencing ptosis.  After a blepharoplasty, the forehead and eyebrows may relax.  The reason is simple.  Generally before surgery, the eyelid fold which is operated on during blepharoplasty rests on the eyelid.  This sends a signal to the forehead and eyebrow lifting muscles to cinch up, lifting the eyelid fold so it does not weigh so heavily on the actual eyelid.  When surgery trims this eyelid fold, there is less tissue weighing on the eyelid.  The signal to lift the forehead and eyebrow decreases and the eyebrow falls, lowering what remains of the eyelid fold.  For most, this difference is small and just not an issue.  In fact, the relaxation of the forehead and eyebrow muscles is possibly the most significant benefit of upper eyelid surgery.  However, for some individuals, the fall in the eyebrow can bring too much of this fold down.  They may feel that they got very little benefit from the eyelid surgery.  Sometimes the answer is removing more upper eyelid skin.  However, sometimes the answer may be a different approach altogether like a forehead lift.

Actual upper eyelid ptosis is another reason why the eye might be heavy after eyelid surgery.  The tendon that lifts the upper eyelid is held in place by loose fibrous tissues that become looser in time as we blink our eyelids.  In some cases this causes an the tendon to slip out of place resulting in a drop in the eyelid position.  In a rare case, the tendon may be loosened by direct damage from surgery.  However, it is probably much more common for eyelid swelling after surgery to simply weigh the eyelid down and cause this delicately held tendon to slip.  As the swelling resolves, some of the heaviness resolves.  However, after about 6 months or a year, if there is no further improvement, surgery may be needed to tighten the upper eyelid tendon.  This surgery is called upper eyelid ptosis surgery.

How can I fix my ptosis?

There are different solutions to this problem, and the right one for you will be determined through a personal consultation that includes a discussion of your goals and a physical exam to identify the core issues that are causing your problem.  There are a few approaches, which include:
•    Removal of excess skin in the upper eyelid to reduce the forehead activation needed to elevate the excess eyelid skin.  This offers an appearance that doesn’t seem surgical.
•    If you need your eyes to appear brighter, you may need a forehead lift in conjunction with eyelid surgery to address all of your facial dynamics.  This stabilizes the face.  It’s important to note that not everyone who needs blepharoplasty also needs a forehead lift, and not everyone with brow heaviness needs a forehead lift.

What if my eyelashes point down?

This is a different kind of ptosis called eyelash ptosis, and it can happen in relation to poor levator fixation of the upper eyelid platform skin.  Eyelid revision surgery by an expert can improve the support for the eyelashes by creating a balanced tension that rotates the eyelashes upward.  This procedure can usually be performed in the office under local anesthesia, and recovery time is similar to standard upper blepharoplasty: 10 to 14 days before you are ready for social situations.

Will nanotechnology make my face look better?

When you are looking for results, seeking out the latest buzz in treatment is not necessarily the option that will yield the best results.  New technologies are new for a reason – they are still being developed and studied, while many less honest marketers piggyback on the work of actual scientists by claiming to use a technology when they really aren’t.  Being one of the first to undergo a treatment is risky; there are basically three things that could happen:

1.    Your results will be as amazing as you’d anticipated.
2.    Your results will be limited, disappointing, or nonexistent.
3.    You may experience negative side effects and complications.

Of course, we all hope for #1 to be us, but since new treatments haven’t had time to be studied, perfected, and tailored to the individual, #2 is more likely.  It’s also possible that the new treatment will harm you in ways that are hard to predict just because we don’t have enough information or experience yet; this could mean you are unlucky #3.

In the case of a cosmetic procedure, a brand-new, unproven treatment probably isn’t the best option.  You are probably better off going with a time-tested, successful treatment like BOTOX, the facelift, or a facial filler.  Many apparently cutting edge technologies that get talked about on the internet or even promoted by cosmetic physicians don’t actually exist.  This is currently the case for Nanotechnology.  It may be fun to speculate that one day, tiny nanobots might treat skin wrinkles from the inside.  However, at present this is the stuff of science fiction.  History has taught use that many of these cutting edge technologies may not prove to be safe and reliable.  Gene therapy is an example of this.  Early gene therapy for infants with severe combined immunodeficiency actually caused leukemia in a very high percentage of  successfully treated infants.  It sometimes makes sense for someone with a terminal condition to participate in new, scientifically-founded research.  However, this calculus does not make sense for someone looking for a cosmetic benefit to their appearance.  At least in a clinical study the potential risks are highlighted and complications are studied.  Unfortunately, this is not always the case with cutting edge cosmetic treatments.  Remember, there are currently many excellent and reliable treatment methods.  Why choose an option that has not stood the test of time or any testing for that matter?
